An average mature plant was estimated to produce 170 g of “marketable substance.” The value of the Canadian crop is uncertain, but has been estimated to be in the billions of dollars annually (Heading 1998; MacLeod 1999).

"Following enactment of the 2014 farm bill provision allowing for growing hemp under certain circumstances, several states have quickly been adopting new state laws to allow for cultivation. To date, more than 30 states or territories have enacted or introduced legislation favorable to hemp cultivation (Figure 6). Other states reportedly considering hemp legislation include Alaska, Arizona, Florida, Georgia, Iowa, Kansas, Massachusetts, Mississippi, New Mexico, South Dakota, Texas, and Wisconsin.53 (The status of state actions regarding hemp is changing rapidly, and information differs depending on source.54)

This article will attempt to present information concerning cannabinoid mechanisms of analgesia, review randomized clinical trials (RCTs) of available and emerging cannabinoid agents, and address the many thorny issues that have arisen with clinical usage of herbal cannabis itself (“medical marijuana”). An effort will be made to place the issues in context and suggest rational approaches that may mitigate concerns and indicate how standardized pharmaceutical cannabinoids may offer a welcome addition to the pharmacotherapeutic armamentarium in chronic pain treatment.


Although hemp can be successfully grown continuously for several years on the same land, rotation with other crops is desirable. A 3- or preferably 4-year rotation may involve cereals, clover or alfalfa for green manure, maize, and hemp. In Ontario it has been recommended that hemp not follow canola, edible beans, soybeans or sunflowers. However, according to Bócsa and Karus (1998), “it matters little what crops are grown prior to hemp.”
Quality is a particular concern, because cannabis plants easily soak up heavy metals from pesticides and other contaminants, Marcu says. If you are buying online, look for a company that documents how it tests its products. (If the website doesn’t indicate this, call and ask.) “Buying from a reputable manufacturer is crucial, because it matters how the plant is cultivated and processed,” Dr. Maroon says. It is important to note that the federal government still considers cannabis a dangerous drug and that the illegal distribution and sale of marijuana is a serious crime. Under the Controlled Substances Act (CSA), marijuana is still considered a Schedule 1 drug. Cultivation and distribution of marijuana are felonies; possession for personal use is a misdemeanor; possession of “paraphernalia” is also illegal. Cultivating 100 plants or more carries a mandatory minimum sentence of five years according to federal statutes. 

If medical marijuana is illegal in a given state, THC levels determine whether a CBD product is illicit or not. In most places, the limit is extremely low. We’re talking under 1 percent THC, with some states opting for a cap as low as 0.3 percent. In this case, the only source that would work is hemp, and CBD products will, therefore, be hemp-derived.

Only since the flower-power-time of the 1960s, the smoking of Cannabis as a recreational drug has become a widely known phenomenon in the Western world. 38 states and Puerto Rico considered legislation related to industrial hemp in 2017. These bills ranged from clarifying existing laws to establishing new licensing requirements and programs.  At least 15 states enacted legislation in 2017 — Arkansas, Colorado, Florida, Hawaii, North Dakota, Nevada, New York, Oregon, South Carolina, Tennessee, Virginia, Washington, West Virginia, Wisconsin and Wyoming. At least four states — Florida, Nevada, New Mexico and Wisconsin — authorized new research or pilot programs. 

There is a general inverse relationship in the resin of Cannabis between the amounts of THC present and the amount of the other principal cannabinoid, CBD. Whereas most drug strains contain primarily THC and little or no CBD, fiber and oilseed strains primarily contain CBD and very little THC. CBD can be converted to THC by acid catalyzed cyclization, and so could serve as a starting material for manufacturing THC. In theory, therefore, low-THC cultivars do not completely solve the problem of drug abuse potential. In practice, however, the illicit drug trade has access to easier methods of synthesizing THC or its analogues than by first extracting CBD from non-drug hemp strains.


Cannabis has mental and physical effects, such as creating a "high" or "stoned" feeling, a general change in perception, heightened mood, and an increase in appetite.[21][22] Onset of effects is within minutes when smoked, and about 30 to 60 minutes when cooked and eaten.[21][23] They last for between two and six hours.[23] Short-term side effects may include a decrease in short-term memory, dry mouth, impaired motor skills, red eyes, and feelings of paranoia or anxiety.[21][24][25] Long-term side effects may include addiction, decreased mental ability in those who started as teenagers, and behavioral problems in children whose mothers used cannabis during pregnancy.[21] There is a strong relation between cannabis use and the risk of psychosis,[26] though the cause-and-effect is debated.[27]
In 2014, President Obama signed the Farm Bill of 2014 into law. This law contained a section that removed hemp from Schedule 1 of the Controlled Substances Act. It also created a legal structure that made cultivation and research of hemp legal in states that wanted to initiate “Pilot Research Programs” into the cultivation and marketing of hemp and hemp-derived products.
And without high-quality trials, experts don’t know how much is best for a given purpose. The staff at Roth’s dispensary told her, “Try some once or twice a day and see what happens.” (Half a dropper’s worth was a good amount for her.) One thing scientists feel confident about is that CBD is not dangerous. It won’t damage vital organs even at doses as high as 5,000 mg a day, Marcu says, and nobody has died from simply overdosing on a cannabis product.

As a result of intensive selection in cultivation, Cannabis exhibits many sexual phenotypes that can be described in terms of the ratio of female to male flowers occurring in the individual, or typical in the cultivar.[28] Dioecious varieties are preferred for drug production, where the female flowers are used. Dioecious varieties are also preferred for textile fiber production, whereas monoecious varieties are preferred for pulp and paper production. It has been suggested that the presence of monoecy can be used to differentiate licit crops of monoecious hemp from illicit drug crops.[22] However, sativa strains often produce monoecious individuals, probably as a result of inbreeding.

Not surprisingly, the data we have are messy. Berenson, in his role as devil’s advocate, emphasizes the research that sees cannabis as opening the door to opioid use. For example, two studies of identical twins—in the Netherlands and in Australia—show that, in cases where one twin used cannabis before the age of seventeen and the other didn’t, the cannabis user was several times more likely to develop an addiction to opioids. Berenson also enlists a statistician at N.Y.U. to help him sort through state-level overdose data, and what he finds is not encouraging: “States where more people used cannabis tended to have more overdoses.”

Moreover, a patient survey conducted by Project CBD, declared that “…cannabis appears to be an effective pain management tool with few negative side effects.” The study went on to say that a “…significant decrease in opiate usage among elderly patients while taking medical cannabis [was observed during trial].” In short, it has been portrayed clearly numerous times through valid and well-publicized clinical studies that cannabis is a practical option in terms of efficient pain management.

The US Drug Enforcement Administration’s online criminal justice statistics for 2000 (cscmosaic.albany.edu/sourcebook/1995/pdf/t440.pdf) shows the following seizures and eradication of plants of C. sativa: 40,929 outdoor plots (2,597,796 plants), 139,580,728 ditchweed (ruderal plants), 2,361 indoor operations (217,105 plants), for a grand total of 2,814, 903 plants destroyed. Impressively, the species was grown in all 50 states (including outdoor seizures in every state except Wyoming)! It is of course impossible to know exactly how much marijuana is cultivated in the United States, and perhaps only 10% to 20% of the amount grown is seized. The profitability of the illegal crop is indicated by a comparison of the cost of a bushel of corn (roughly $2.50) and a bushel of manicured marijuana (about $70,000; it has been suggested that prices range from $500 a pound, for low-quality marijuana, to more than $5,000 a pound for “boutique” strains like “Northern Lights” and “Afghan Kush”). According to a National Organization for the Reform of Marijuana Laws (NORML) (mir.drugtext.org/marijuananews/marijuana_ranks_fourth_largest_c.htm) marijuana is at least the fourth most valuable crop in America, outranked only by corn, soybeans, and hay. It was estimated that 8.7 million marijuana plants were harvested in 1997, worth $15.1 billion to growers and $25.2 billion on the retail market (the wholesale value was used to compare marijuana to other cash crops). One of the most curious uses of hemp is as a fence to prevent pollen transfer in commercial production of seeds. Isolation distances for ensuring that seeds produced are pure are considerable for many plants, and often impractical. At one point in the 1980s, the only permitted use of hemp in Germany was as a fence or hedge to prevent plots of beets being used for seed production from being contaminated by pollen from ruderal beets. The high and rather inpenetrable hedge that hemp can produce was considered unsurpassed by any other species for the purpose. As well, the sticky leaves of hemp were thought to trap pollen. However, Saeglitz et al. (2000) demonstrated that the spread of beet pollen is not effectively prevented by hemp hedges. Fiber (i.e. tall) cultivars of hemp were also once used in Europe as wind-breaks, protecting vulnerable crops against wind damage. Although hemp plants can lodge, on the whole very tall hemp is remarkably resistant against wind.

"There is little information about DEA’s permit process and on facilities that are licensed to grow hemp, even for research purposes. Previously reports indicated that DEA had issued a permit for an experimental quarter-acre plot at the Hawaii Industrial Hemp Research Program during the period from 1999 to 2003 (now expired).69 Most reports indicate that DEA continues to be reluctant to grant licenses to grow hemp, even for research purposes.70 Recent indications are that some land grant university researchers may have been granted licenses to conduct hemp research under certain conditions.71

One of CBD’s chief properties is its anticonvulsant nature. Clinical trials have shown that CBD is effective at reducing seizures in children, and the FDA is likely to approve Epidiolex, a pharmaceutical-grade version of CBD for this use, in summer 2018. Although CBD has been documented as an antiepileptic since 1881, CBD’s anticonvulsant mechanisms still remain unclear. Not enough studies have been conducted to understand this relationship fully. One possible explanation for CBD’s neuroprotective effects is its interaction with NMDA receptors, which play a key role in the overly active neuron activity that is a hallmark of epilepsy.

The question of whether heteromorphic sex chromosomes are indeed present is most conveniently answered if such chromosomes were clearly visible in a karyotype. Cannabis was one of the first plant species to be karyotyped; however, this was in a period when karyotype preparation was primitive by modern standards (see History of Cytogenetics). Heteromorphic sex chromosomes were reported to occur in staminate individuals of dioecious "Kentucky" hemp, but were not found in pistillate individuals of the same variety. Dioecious "Kentucky" hemp was assumed to use an XY mechanism. Heterosomes were not observed in analyzed individuals of monoecious "Kentucky" hemp, nor in an unidentified German cultivar. These varieties were assumed to have sex chromosome composition XX.[32] According to other researchers, no modern karyotype of Cannabis had been published as of 1996.[33] Proponents of the XY system state that Y chromosome is slightly larger than the X, but difficult to differentiate cytologically.[34]

With marijuana, apparently, we’re still waiting for this information. It’s hard to study a substance that until very recently has been almost universally illegal. And the few studies we do have were done mostly in the nineteen-eighties and nineties, when cannabis was not nearly as potent as it is now. Because of recent developments in plant breeding and growing techniques, the typical concentration of THC, the psychoactive ingredient in marijuana, has gone from the low single digits to more than twenty per cent—from a swig of near-beer to a tequila shot. 

With the passage of the 2018 Farm Bill, America will have a chance to counter China’s massive influence in the cannabis hemp industry. But it will mean playing catch-up for the foreseeable future. Currently, hemp cultivation techniques in America lag far behind other crops—it still has to be harvested by hand even. Even though marijuana is legal in China, the country has funded research into the plant and its cultivation, placing it miles ahead of other countries.
Subsequent studies were carried out in different countries, which confirmed the results found in the Zammit et al. (2002) study, showing that those clinically dependent on cannabis by 18 years of age had an increased risk of later developing psychotic symptoms (Fergusson, Horwood, & Swain-Campbell, 2003). Cannabis users were also more likely to develop schizophreniform disorder (Arseneault et al., 2002), and the dose–response relationship found in the first study was confirmed (Henquet et al., 2005).
Public health also takes various actions to limit the health disparities between different areas of the country and, in some cases, the continent or world. One issue is the access of individuals and communities to health care in terms of financial, geographical or socio-cultural constraints to accessing and using services.[54] Applications of the public health system include the areas of maternal and child health, health services administration, emergency response, and prevention and control of infectious and chronic diseases.
CBD’s action within the brain and body is quite complicated. To date, scientists have discovered more than a dozen different mechanisms of action, or ways that CBD affect us. It’s very likely that the beneficial effects of CBD are a result of the total of its activation of all of these biological pathways, not a single one in particular. Much more research is needed to fully understand the mechanisms by which CBD relieves ailments such as anxiety and seizures.

THC and its major (inactive) metabolite, THC-COOH, can be measured in blood, urine, hair, oral fluid or sweat using chromatographic techniques as part of a drug use testing program or a forensic investigation of a traffic or other criminal offense.[53] The concentrations obtained from such analyses can often be helpful in distinguishing active use from passive exposure, elapsed time since use, and extent or duration of use. These tests cannot, however, distinguish authorized cannabis smoking for medical purposes from unauthorized recreational smoking.[150] Commercial cannabinoid immunoassays, often employed as the initial screening method when testing physiological specimens for marijuana presence, have different degrees of cross-reactivity with THC and its metabolites.[151] Urine contains predominantly THC-COOH, while hair, oral fluid and sweat contain primarily THC.[53] Blood may contain both substances, with the relative amounts dependent on the recency and extent of usage.[53]

The genus Cannabis was formerly placed in the nettle (Urticaceae) or mulberry (Moraceae) family, and later, along with the genus Humulus (hops), in a separate family, the hemp family (Cannabaceae sensu stricto).[45] Recent phylogenetic studies based on cpDNA restriction site analysis and gene sequencing strongly suggest that the Cannabaceae sensu stricto arose from within the former family Celtidaceae, and that the two families should be merged to form a single monophyletic family, the Cannabaceae sensu lato.[46][47]
